Insurance cards or payments are required at the time of vaccination.  Most insurance plans are accepted.  If insurance does not cover or there is no insurance to bill the cost is as follows;

      High-Dose (age 65+): $65.00,

Flu Blok (age 18–64): $65.00,

Regular Flu (6 months and older): $40.00

Further details are attached to this cover letter. This Public Notice requires a 15-day posting period for comments from the public beginning on the date the notice was published.
